      Standard Mail Permit must have 200 or more pieces to qualify

      First Class Mail Permit must have 500 or more pieces to qualify

      All necessary supplies (envelopes, postcards, etc.) will be
            supplied or paid for in advance

      User Data on CDs, discs, or Email must be provided in:
            .dbf, .xls, .mdb (Instant Impact), or  text comma delimited         

      Pickup arrangements must be made in advance

      Charges for out-of-area pickup will apply

      All unused supplies will be returned

      All information is held in strictess confidence

      Data Processing Services: $25 / hr

      Create Database for Mailing List: 25 cents per entry
